FAQ for New or Prospective Patients

  • Yes, there will be a number of forms to fill out during the check-in process. We will do our best to send these to you digitally, and before your appointment so you have time to fill them out. This also helps us expedite your visit, so you can come in, be seen, get answers, and have a plan of care.

    Please be sure to fill out at thoroughly as possible.

  • Bring your government-issued ID, insurance card, and any relevant medical records or information you think would be helpful us to understand your reason for visit as completely as possible.

  • This depends on your insurance carrier. Our clinic does not have a policy against self-referrals, but please be sure your insurance company will cover the visit cost if you decide to go this route.

    Please check with your insurance carrier prior to self-referral to avoid any surprise costs or denial of coverage.

  • Not at all. We understand that many folks are still practicing COVID-safe habits, may be immunocompromised, or simply feel safer if those around them masked, especially in a physician’s office where there may be ill people coming and going. Please DO NOT HESITATE to ask or request that we mask.

  • Our office requires co-pay upfront prior to rendering services during a clinic or consultation visit.

    For self-pay and uninsured patients, payment will be required in full after clinic services are rendered. Any in-office diagnostic procedure will be discussed prior to performing to eliminate surprises. Depending on the office procedure to be performed, it may require scheduling to ensure enough time for the procedure.

    For out-of-network patients, payment will be required in full after clinic services are rendered, and reimbursement from our clinic will be sent to you via check or e-payment after submission to your insurance carrier.
